Indications for intravenous gammaglobulin therapy in inflammatory myopathies.

چکیده
Polymyositis (PM) and dermatomyositis (DM) are inflammatory muscular diseases of unknown cause. An immune mechanism may cause these inflammatory myopathies.'2 The mainstay of therapy in inflammatory myopathies is corticosteroids.3 In patients who are resistant to corticosteroids, many therapies are available: immunosuppressive drugs, plasmapheresis, or total body irradiation.4 These therapies are not always effective and may be responsible for certain serious side effects. Immunomodulatory therapy with intravenous immunoglobulins (IVIg) has been shown to be useful in a number of diseases featuring disordered immunoregulation.5 Consequently, we first administered IVIg to patients with chronic refractory inflammatory myopathies in an uncontrolled trial6 7 with favourable results. We describe our experience of IVIg in PM/DM.

Current pharmacological treatment of idiopathic inflammatory myopathies.
The idiopathic inflammatory myopathies are uncommon and heterogeneous disorders. Their classification is based on distinct clinicopathologic features. Although idiopathic inflammatory myopathies share some similarities, different subtypes may have variable responses to therapy, so it is very important to distinguish the correct subtype. There are few randomised, double blind placebo controlled ...
متن کاملIntravenous immunoglobulin therapy for resistant ocular Behçet's disease.
AIMS The present report was aimed at finding out whether gammaglobulin could have a role in treating ocular Behçet's disease (BD) refractory to accepted medical therapy. METHODS Six eyes of four patients with ocular BD refractory to steroids and cyclosporin A were treated with a course of intravenous gammaglobulin and followed up for their response to treatment. RESULTS All six eyes of all ...
متن کاملMaintenance Treatment of Adults With Chronic Refractory Immune Thrombocytopenic Purpura Using Repeated Intravenous Infusions of Gammaglobulin
Intravenous infusion of gammaglobulin (IVGG) has been extensively used in the treatment of immune thrombocytopenic purpura (ITP) in adults to acutely raise the platelet count but not as a maintenance therapy. This report describes the maintenance treatment of adults with chronic ITP using repeated infusions of 800 to 1 .000 mg/kg of IVGG. Sixteen of 40 patients were able to discontinue all ther...

The mainstay of pharmacologic therapy in patients with inflammatory myositis is corticosteroid and sometimes immunosuppressive drugs. However, because patients sometimes become refractory to these drugs and since these drugs have potential short and long-term toxicities, an alternate therapy is highly desirable. Therefore, a pilot study was initiated using intravenous gammaglobulin(IV...
متن کاملIntravenous gammaglobulin as rescue therapy in a patient with sickle cell and septic shock.
INTRODUCTION We present a case involving a patient with sickle cell and hyposplenism, in which refractory septic shock quickly responded after the infusion of intravenous gammaglobulin (IV-GG) given as an adjuvant-rescue therapy CASE DESCRIPTION A 30-year-old African-American female with history of Sickle Cell disease was admitted for acute chest syndrome, septic shock and respiratory failure...
